Analysis of Lymphocytic Activity Marker of the Palatine and Pharyngeal Tonsil in Children with Paranasal Sinusitis and Otitis Media with Effusion

Abstract
Background and objectives: In patients with palatine tonsillar hypertrophy and adenoidal hypertrophy, it is known that sinusitis and OME has aerodynamical, bacteriological, immunological interaction, but there are still many debates on their immunological roles. The aim of this study was to investigate the activity marker of lymphocyte of palatine tonsil and adenoid. We may estimate of local immunological role of palatine tonsil and adenoid in pathogenesis of hypertrophy, sinusitis, and otitis media with effusion

Method: Tissues were collected from 20 pediatric patients who undergo tonsillectomy and adenoidectomy. CD3/HLA-DR, CD25 were used for T lymphocyte activity marker and PCA-1 was used for B lymphocyte activity marker. By flow cytometric analysis, expression of surface marker was evaluated for adenoidal and tonsillar hypertrophy patients and control, OME pateints and control, paranasal sinusitis patients and control.

Results: In adenoid and tonsillar hypertrophy patients and in paranasal sinusitis patients, activity markers have no different from control group. In OME pateints, PCA-1 has more increased activity than non-OME group in tonsil (p=0.002) and adenoid (p=0.028).

Conclusion: In adenoid and tonsillar hypertrophy patients, and in sinusitis patients group, it is supposed that aerodynamic and bacteriologic factor are main role. In our study, T lymphocytes activity difference were not found. And humoral immunity may have main action in OME.
